The type of information we collect
a) Customers and Potential Customers
We will only collect personal information necessary for us to supply
you with those products, services or information you order or request
from us. This may include – depending on the type of product,
service or information you require:
(i) Your photo, in digital or hard copy format,
(ii) Your name, date of birth, telephone number, facsimile number,
postal Address, email address,
(iii) Your employer’s name and any identifying employee codes
or numbers,
(iv) Your school name, year level, class and identifying numbers or
codes,
(v) Your school’s crest or logo
(vi) Company and Event logos,
(vii) Any clubs and or organizations to whom you belong, and any identifying
numbers or codes,
(viii) Personal, business and financial references for the purposes
of allowing you Trade Credit,
(ix) Financial information relevant to allowing you Trade Credit
(x Any other data which may from time to time be required providing
the data is specifically for the purpose of supplying you with those
products, services or information you order or request from us.
The information we collect will be the information
necessary to supply you with those products, services or information
you order or request from us, and will not be used for any other
purpose.
We will not collect sensitive information such as ethnic origin,
religious or philosophical beliefs, sexual preferences or health.
We will collect, but only to the extent necessary to meet your request
for our products or services, your membership of political, trade
or professional association. For example, production of name tags
for the XYZ Association would require us to record your membership
of that association.
b) Employees
We will collect information to assist employees in their application
for an employment position with us. This information may include:
(i) Your employment history and contact details,
(ii) Any personal and professional referees,
(iii) Your educational history,
(iv) Your affiliation with any trade or professional associations.
c) Trade Suppliers
We will collect financial information from you to the extent necessary
to facilitate our financial dealings with you. This may include:
(v) Your bank name and account number,
(vi) Referees to assist in the assessment of your business.
2) How we collect the information
(a) Customers and Potential Customers
Generally, we collect personal information from you at the time
you request supply of our products, services or information. This
can be by telephone, facsimile, email, internet or other direct
communications between you and us. For Photo ID Cards, and or Name
Tag Cards, and or Membership Cards, we can also obtain information
about you from third parties.
The information obtained is the information necessary for us to
supply you with our products, services or information you have requested,
or a third party acting on your behalf has requested.
The third parties include, but are not restricted to:
(i) Schools issuing you with an Identity Card,
(ii) Employers or other Organisations issuing you with an Identity
Card,
(iii) Organisors of Conferences, Meetings, Trade Shows, Expositions
who want you to wear an identity card during their event.
(iv) Sporting Clubs, Professional and Trade Associations, Hobby
Clubs and any other organization to whom you belong and want you
to possess an identity card signifying your membership,
(v) Third parties may also request us to supply data about you,
including your photographic image, on CD-ROM.
(b) Employees
We collect personal information from you at the time you apply for
a position with us. This can be by telephone, facsimile, email,
internet or other direct communications between you and us and any
third party referee you nominate in your employment application.
(b) Trade Suppliers
We collect personal information from you at the time we apply for
Trade Credit from you, and or commence a trading relationship with
you. This can be by telephone, facsimile, email, internet or other
direct communications between you and us and any third party referee
you may have nominated.
3) Why we collect the information
We collect this information in order to undertake and complete the
many transactions we have with our customers, suppliers, employees
and other parties with whom we do business, and to administer the
many products and services we provide and to maintain the business
relationships we have that enable us to carry on our business.
Personal information may also be used for keeping
our customers informed of the many products and services we have,
which may include using it for direct marketing.
4) How we store the information
We store personal information on a combination of secure computer
storage facilities and paper based files and other records. We have
taken a number of steps to protect the personal information we hold
from misuse, loss and unauthorised access, modification or disclosure.
We will take reasonable steps to securely destroy
or permanently de-identify personal information when we no longer
need it.
We use generally accepted technology and security so that we are
satisfied that your information is transmitted safely to us through
the internet or other electronic means.
5) How you can ensure our information is accurate
We will take steps to ensure that personal information we hold is
accurate, complete and up-to-date. If you think that we hold information
that is incorrect in any way, please contact us and we will take
all reasonable steps to remedy the error to your satisfaction.
Following a request, we will provide you with a copy of personal
information we hold about you in accordance with our obligations
under the Privacy Act. We may charge a fee for retrieving this information
(we will inform you of the fee before providing the information).
Please note that there are some circumstances set out in the Privacy
Act where we may refuse your request.
6) Do we give your information to anyone else?
We may disclose personal information about our customers and potential
customers, including card-holders with personal data, to third party
contractors, service providers and customers and other third parties
who help us operate our business. We require these third parties
to bound by, and to abide by, the National Privacy Principles in
the Privacy Act (Cth) 1988 (“Privacy Act”).
Specifically, we may provide personal information to the Private
and Public Operators of the Public Transport System in any state
of Australia. The information provided will be restricted to the
information we collect that is required by us to produce an Identity
Card, and will include name, photo, date of birth, school you attend,
year level and any school identifying codes or numbers.
We will provide personal information if required to do so for legal
reasons, such as a court order, or other legally enforceable demand(s)
to do so.

We may update our policies and this Privacy Policy Statement from
time to time. The latest version is published on our web site at
www.ezitag.com.au and is available in print at our office, or by
contacting us shown below.
